Care Management and Social Work – Patient Care Services
According to the U.S. Department of Veterans Affairs, “CM/SW programs provide a vast array of clinical service, which address the needs of wounded, injured and ill Servicemembers, Veterans, their family members, and family caregivers.” There are six VA CM/SW programs, including:
- Caregiver Support Program
- Partner Violence (DV/IPV) Assistance Program
- Fisher House & Family Hospitality Program
- Social Work
- Military2VA Case Management Program
- VA Liaison for Healthcare Program
